Quick answer ·
You barely notice it on the court. It shows up the next morning, reaching for a cup. What is usually happening underneath, and where it really started.
On the court you barely notice it. The paddle weighs almost nothing, the rallies keep you moving, and your body never really gets a chance to complain. It turns up later, doing something small. Taking a cup down from a shelf. Getting an arm into a coat sleeve. Pulling a shirt off over your head.
And it is rarely dramatic. Most people do not arrive at the word pain for months. What they notice first is that they have started doing things differently. Turning the whole body instead of lifting the arm. Reaching with the other hand without having decided to. Letting the overhead serve quietly drop out of the rotation.
That swap is worth paying attention to, because it usually means something above the shoulder has stopped helping.
Your arm has help. You have just never had to notice it.
Raise your arm all the way overhead and only about two thirds of that movement comes from the shoulder joint itself. The last third comes from the shoulder blade, which turns on your back as you lift. Its bottom tip swings out, away from your spine, and the socket tilts until it faces the ceiling.
So your arm does not really climb up there on its own. The socket goes out to meet it.
There is a condition attached, though. The blade has to turn while it stays down. If it simply rides up toward your ear instead, your arm still goes overhead, but it goes there as a shrug. Same arm in the air, a very different machine underneath. And every degree the blade fails to deliver, the shoulder joint quietly covers.
And the blade is not on its own either. It rides on your ribs.
This is the part almost nobody hears. The shoulder blade has no proper joint of its own. It floats on the back of your ribcage, held in place by muscle. The main muscle that turns it begins on your upper ribs, wraps around your side, and grips the inner edge of the blade.
Which means your ribs are its starting block. When the ribcage settles and flattens, that muscle loses the platform it pulls from. The blade is not weak. It just has nowhere good to go.
Now hold a ready position for two hours.
The court is small, so the ball comes back fast and often. You stand low, chest down, paddle up, waiting. Breathing goes shallow and climbs into the top of the chest, the way it always does when you are braced and waiting for something. The ribs stay flat. The blade lifts instead of turning. And the shoulder joint ends up covering the whole reach, a few hundred times in an afternoon.
The socket your arm sits in is shallower than most people imagine, closer to a saucer than a cup, and four small tendons spend the entire game keeping the head of the arm centred on it. Hand them the whole job often enough and eventually they let you know.
The pain arrives at the shoulder. The shortage started further upstream.
Three things you can check tonight
These are observations, not exercises. You are looking for a difference between sides, not a score.
One. Rest both hands on your lower ribs, at the sides, and breathe in. If your hands move outward, the ribs are opening. If only your shoulders rise, your breath has climbed.
Two. Face a mirror and raise both arms slowly. Watch only the first part of the way up. If one shoulder hitches toward your ear before the arm really moves, that blade is lifting instead of turning.
Three. Stand with your back to a wall, keep your lower back against it, and lift both arms toward the wall. If your low back peels away or a shoulder shrugs before your arms get there, the mid-back and ribs are not giving you what the movement needs.
What you actually want back is the not-thinking
Most people who come in for this do not lead with pain. They describe the calculating. Choosing which arm to reach with. Skipping the serve. Thinking about tomorrow morning in the middle of a game.
Getting rid of that is the real goal, and it is a better one than a pain score, because it is the thing you would notice on an ordinary Tuesday.
We look at it in the order your body uses
Breathing first, because everything above it leans on it. Then the ribcage, and the segments of your mid-back that have stopped moving. Then how the shoulder blade travels, and whether it can stay down while it turns. The shoulder joint itself comes last, and by the time we get there it is usually asking for a lot less than it was.
You will also leave knowing what you are looking at. If the mid-back turns out to be the piece that gave way, you should be able to feel that for yourself rather than take our word for it.
Reaching for the ball should not take any thought. If it has started to, it is worth a look before the season gets long.
