• Information about a health condition or symptom you describe to us before you become a patient — what hurts, how long it has hurt, whether an auto accident or work injury is involved. Collected to answer your question and to determine whether we are the right clinic for you.
• Information about your interest in chiropractic, rehabilitation, or related care, including the appointment type you request. Collected to schedule you correctly.
• Contact information submitted together with the above — name, phone number, email address, preferred language. Collected to reply to you.

Under the My Health My Data Act you may:

Our response. We reply within 45 days. If we need more time we will tell you within those 45 days and take up to 45 more. Requests are free unless they are repetitive or excessive. When you ask for deletion, we delete the data from our records and backups and notify our processors, affiliates, and any third party that received it, within 30 days.

처리 절차. 요청일로부터 45일 이내에 회신합니다. 추가 기간이 필요한 경우 45일 이내에 그 사실을 알려드리고 최대 45일을 연장합니다. 반복적이거나 과도한 요청이 아닌 한 비용은 청구하지 않습니다. 삭제를 요청하시면 저희 기록과 백업에서 해당 정보를 삭제하고, 위탁업체·계열사·정보를 제공받은 제3자에게 30일 이내에 통지합니다.

If we decline. You may appeal by replying to our decision or by writing to the address above. We respond to appeals within 45 days and explain the reasons. If the appeal is denied, you may submit a complaint to the Washington State Attorney General at www.atg.wa.gov/file-complaint.

If this policy changes, the updated version is posted here with a new effective date, and we will not apply the change retroactively to previously collected consumer health data without your consent.
