You already know how the night goes. A late dinner, or a heavy one. You lie down, and within the hour there is that sour warmth climbing up behind your breastbone.

Here is something almost no one mentions. The valve that holds your stomach closed is not only a valve. Part of it is a muscle you use every time you breathe.

One door, two rings

Where your esophagus passes down into your abdomen, it goes through a narrow slit in your breathing muscle — the diaphragm. That opening sits at your tenth thoracic vertebra, T10: about the middle of your back, a little left of centre.

Two things guard it. Inside, a ring of smooth muscle in the wall of the esophagus itself. Outside, the fibres of your diaphragm wrap around it like a noose — that is the anatomists' own word — and each time that muscle contracts, the tube is drawn a little more firmly shut.

Your diaphragm is not free-floating. It anchors to your lower ribs, and by two long tendons called the crura it reaches down and fastens onto the front of your spine. In the most literal sense, it hangs there.

So when the mid-back stiffens and the rib cage stops moving well — after years at a desk, or simply after we stop moving the way we once did — the diaphragm loses some of its travel. It sits flatter. And the outer ring at T10 does its work with less conviction than it could.

Posture adds a second push. A slumped, compressed torso raises the pressure inside the abdomen — and pressure rising from below is precisely what a tired door has to hold against.

What the research shows

The outer ring is a skeletal muscle, so researchers asked the obvious question: can it be trained? Across ten randomised trials, diaphragmatic breathing training produced a modest but real improvement in reflux symptom scores.

How we work

First we look at the frame: how your mid-back and lower ribs actually move, and where they have stopped. Then we free what has stopped, so the diaphragm has room to travel again. Then we retrain the breath, so the ring at T10 is working with you rather than sitting idle.

Chest discomfort can come from the heart, not the stomach, and the two feel remarkably alike. Call 911 if chest pain is new or severe, or comes with sweating, nausea, or pain spreading into your arm, jaw, or back.
