Someone has probably already told you that you look fine.

You know what that sentence costs. The hardest part of this diagnosis is so rarely the part anyone can see — the fatigue that arrives at two in the afternoon like a weight being set down on you, the words that stop being available mid-sentence, the leg that turns heavy in a warm room. From the outside, nothing at all. From the inside, a full day's work just to get through the day.

I want you to know that I believe that part. Before anything else.

Let me get the honest part out of the way early

Chiropractic care doesn't treat multiple sclerosis. It can't rebuild myelin or change the course of the disease, and nothing I do should ever stand in the place of your neurologist — or become a reason to pause a medication you've been prescribed.

I know that's not what anyone hopes to read near the top of a page like this. I put it here because I'd like you to be able to believe everything that comes after it. You've probably had your fill of people promising more than they can give.

이런 글의 첫머리에서 듣고 싶은 이야기가 아니라는 걸 압니다. 그럼에도 여기에 두는 이유는, 이 뒤에 이어지는 이야기만큼은 마음 놓고 믿으셨으면 하기 때문입니다. 줄 수 있는 것보다 많은 걸 약속하는 사람들은 이미 충분히 겪으셨을 테니까요.


What's actually happening: a postal service with damaged roads

Your brain and spinal cord are a central post office. Every movement, every sensation, every command leaves as an electrical letter. The road it travels is the axon, and the axon is wrapped in a smooth insulating pavement called myelin — that pavement is why your signals are fast. It lets the current leap ahead instead of crawling.

In MS, the immune system mistakes that pavement for an intruder and strips it away. Repair crews do come, but the patch never quite matches, and what's left behind is a hard scar — a plaque. That scar is the sclerosis in the name. Letters now arrive late, or garbled, or not at all. And because the damage happens in different places at different times, the symptoms scatter: an eye one year, a leg the next.

None of that scattering is you being inconsistent. It's the map of where the roads happen to be broken.

다발성 경화증에서는 면역 체계가 이 포장을 침입자로 착각해 벗겨냅니다. 복구팀이 오긴 하지만 덧댄 자리가 원래만큼 매끄럽지 않아, 그 자리에 딱딱한 흉터가 남습니다. 이 흉터가 병 이름 속 '경화'입니다. 이제 편지는 늦게 오거나, 뭉개져 오거나, 아예 오지 않습니다. 손상이 서로 다른 자리에서 서로 다른 시기에 생기니 증상도 흩어집니다. 올해는 눈, 다음 해는 다리 하는 식으로요.

증상이 그렇게 흩어지는 건 환자분이 일관성 없는 사람이라서가 아닙니다. 어느 길이 끊겼는지를 보여주는 지도일 뿐입니다.


The second injury nobody names

Here's what I see in the room, over and over.

The lesion is one thing. What your body quietly does to survive the lesion is a second thing — and it's usually the second one that hurts.

A leg that drags a little makes the opposite hip work double. A body braced against falling stops rotating through the mid-back. A ribcage held tight stops breathing fully, so the neck and shoulders take over the job of getting air in. Every one of those was a sensible decision at the moment your body made it. Held for months, they turn into stiffness, one-sided back pain, headaches, and a walk that costs far more energy than it should.

Then the loop closes: fatigue → less movement → lost strength → every step costs more → more fatigue.

That loop isn't autoimmune. It's mechanical. Which is another way of saying it can still change.

진료실에서 반복해서 보는 장면이 있습니다.

병변은 병변대로 있습니다. 그런데 그 병변을 버텨내기 위해 몸이 조용히 해온 일이 따로 있고, 실제로 아픈 건 대개 그쪽입니다.

한쪽 다리가 조금 끌리면 반대쪽 엉덩이가 두 배로 일합니다. 넘어지지 않으려 몸에 힘을 주면 등 가운데가 돌아가지 않습니다. 갈비뼈가 굳으면 숨이 얕아지고, 그러면 목과 어깨가 대신 숨을 쉬기 시작합니다. 하나하나는 몸이 그 순간에 내린 합리적인 선택이었습니다. 다만 몇 달을 그렇게 지내면 뻣뻣함이 되고, 한쪽만 아픈 허리가 되고, 두통이 되고, 걷는 데 필요 이상으로 힘이 드는 몸이 됩니다.

그리고 고리가 닫힙니다. 피로 → 덜 움직임 → 근력 손실 → 한 걸음이 더 비싸짐 → 더 심한 피로.

이 고리는 자가면역이 아닙니다. 기계적인 문제입니다. 다르게 말하면, 아직 달라질 수 있다는 뜻입니다.


Who carries which part · 어느 자리를 누가 맡는지

The disease itself is watched and treated by your neurologist, and I'm honestly glad it sits with them. It means someone is keeping careful watch on that part while we put our attention somewhere else. If anything I suggest doesn't fit their plan, theirs is the one that stands.

병 자체는 신경과 선생님이 지켜보고 치료해 주십니다. 그 자리가 그분들께 있다는 게 저는 다행이라고 생각합니다. 누군가 그쪽을 꼼꼼히 보고 계시니 저희는 다른 곳에 마음을 쓸 수 있으니까요. 혹시 제 제안이 그분들의 계획과 어긋난다면, 기준은 언제나 그쪽입니다.

What we work on together here is everything your body built to cope. Stiff joints. A hip quietly doing two jobs. A mid-back that has stopped rotating. A ribcage that has forgotten how to open. A trunk that can't quite hold you steady any more, and a sense of balance that narrowed without anyone mentioning it. None of that is the disease. All of it is ordinary, all of it deserves attention, and all of it answers to patient hands and patient training.

"Rest and conserve" was the old advice. It turned out to be wrong.

For a long stretch of the last century, people with MS were told to avoid exertion — that exercise would burn through a limited reserve or provoke the disease. That guidance has since been turned around. Structured movement is now understood as part of living well with MS: better strength, steadier walking, better mood, and — the part that surprises almost everyone — less fatigue rather than more.

If you were told the opposite years ago, you weren't being careless. You were following the best advice anyone had at the time.

The one condition is that it has to be built for your body, not borrowed from a general fitness plan. That's the whole job, really.

"쉬면서 아끼세요"는 옛날 조언이었고, 결국 틀린 조언이었습니다

지난 세기의 꽤 오랫동안, 다발성 경화증 환자는 무리하지 말라는 말을 들었습니다. 운동이 한정된 에너지를 태우거나 병을 자극한다고 여겼기 때문입니다. 지금은 그 조언이 뒤집혔습니다. 계획된 움직임은 이제 이 병과 잘 지내기 위한 한 부분으로 봅니다. 근력이 늘고, 걸음이 안정되고, 기분이 나아지고, 그리고 거의 모든 분이 놀라시는 부분인데 피로가 더 심해지는 게 아니라 덜해집니다.

혹시 오래전에 반대로 들으셨다면, 그건 부주의하셨던 게 아닙니다. 그때 있던 가장 좋은 조언을 따르신 것뿐입니다.

단 하나의 조건은, 일반 운동 프로그램을 빌려오는 게 아니라 지금 이 몸에 맞게 설계되어야 한다는 것. 사실 그게 일의 전부입니다.


Four rules we train by · 저희가 지키는 네 가지 원칙

i. Heat is the first thing we think about, not the last. When core temperature rises, an already-damaged signal slows further and symptoms surge — vision blurs, a leg gets heavy. It's frightening the first few times, and it's almost always temporary; it settles as you cool. So: a cool room, short bouts, cold water within reach, earlier in the day when we can. Heat is something we manage together, not something you have to be afraid of.
체온이 오르면 이미 손상된 신호가 더 느려져 증상이 확 올라옵니다. 처음 몇 번은 놀라실 만하고, 거의 언제나 일시적이며, 몸이 식으면 가라앉습니다. 서늘한 공간에서, 짧게, 찬물을 곁에 두고, 가능하면 이른 시간에. 열은 함께 관리하는 것이지, 무서워하셔야 할 것이 아닙니다.

ii. We train inside the fatigue budget. This fatigue isn't ordinary tiredness, and it doesn't answer to willpower — so please don't let anyone tell you to push through it. We never train to failure. We stop with something left, break the work into short pieces with real rest between, and judge a session by how you feel the next morning rather than by how hard it felt.
이 피로는 평범한 피곤함이 아니고, 의지로 이겨낼 수 있는 것도 아닙니다. 그러니 참고 밀어붙이라는 말은 듣지 않으셔도 됩니다. 한계까지 가지 않습니다. 조금 남기고 멈추고, 짧게 나눠 하고, 사이에 제대로 쉽니다. 잘한 운동인지는 그날 얼마나 힘들었는지가 아니라 다음 날 아침 몸이 알려줍니다.

iii. We start on the floor, then stand up. Lying on the back, on the side, on all fours — developmental positions let us rebuild breathing and deep trunk control somewhere there's no fall risk at all. The diaphragm is the floor of that whole system; when it starts moving again, the neck and shoulders can finally put down a job that was never theirs. Only once the trunk holds do we take the work upright.
바로 눕기, 옆으로 눕기, 네 발 자세. 발달 자세를 쓰면 넘어질 위험이 전혀 없는 자리에서 호흡과 깊은 몸통 조절부터 다시 만들 수 있습니다. 이 시스템 전체의 바닥은 횡격막입니다. 횡격막이 다시 움직이기 시작하면, 목과 어깨는 그제야 원래 자기 몫이 아니었던 일을 내려놓을 수 있습니다. 몸통이 버티기 시작한 다음에야 선 자세로 올라갑니다.

iv. Balance gets trained on purpose. Balance isn't a bonus that shows up when you get fitter — it's its own skill, and in MS it's the one carrying the most weight, because a single fall can undo months. So it gets dedicated, supported, gradually harder work, with something to hold always within reach and never needed by surprise.
균형은 체력이 좋아지면 따라오는 덤이 아닙니다. 그 자체로 하나의 기술이고, 이 병에서는 걸린 게 가장 큰 항목입니다. 한 번 넘어지면 몇 달이 되돌아가니까요. 그래서 따로, 붙잡을 것을 늘 손 닿는 곳에 두고, 조금씩 어렵게 훈련합니다. 갑자기 필요해지는 일은 없도록요.


Where the spine fits in

Hands-on work here has one job: give movement back to joints that have locked down and release the tissue that shortened around them, so that training has somewhere to go. A hip that won't extend can't be strengthened into a normal stride. A mid-back that won't rotate will keep handing its work to your neck.

And technique gets chosen for you, not for a textbook. Spasticity, bone density, medication history, where your lesions sit — all of it changes what's appropriate, usually toward gentler, lower-force work, sometimes toward leaving a region alone entirely. Whenever your neurology team has a view on any of it, I want to hear it. Not as a box to tick — because you're better off when the people looking after you are actually talking to one another.

척추는 어디에 들어가나요

여기서 손으로 하는 치료의 역할은 하나입니다. 잠겨버린 관절에 움직임을 돌려주고, 그 주변에서 짧아진 조직을 풀어, 훈련이 들어갈 자리를 만드는 것. 펴지지 않는 고관절은 아무리 근력을 키워도 정상 보행이 나오지 않고, 돌아가지 않는 등은 계속 자기 일을 목에게 떠넘깁니다.

그리고 기법은 교과서가 아니라 환자분에게 맞춰 고릅니다. 경직 정도, 골밀도, 복용 이력, 병변의 위치에 따라 적절한 방법이 달라집니다. 대개는 더 부드럽고 힘을 덜 쓰는 쪽으로, 때로는 특정 부위를 아예 건드리지 않는 쪽으로요. 신경과 팀에서 이 부분에 대해 의견이 있으시면 저는 언제든 듣고 싶습니다. 절차를 밟기 위해서가 아니라, 환자분을 돌보는 사람들이 서로 이야기를 나눌 때 환자분께 가장 좋기 때문입니다.


One thing I'd ask you to keep · 한 가지만 기억해 주세요

A hard day is not the same thing as a relapse.

Symptoms that rise with heat, effort, an infection or a bad night's sleep and then settle within hours are usually a temporary flare. But new or clearly worsening neurological symptoms that carry on past a day, with no fever or overheating to explain them, are a different matter — and that's when I'd want you sitting in front of your neurologist that same week.

  • A change in bladder or bowel control · 소변이나 대변 조절의 변화

Bring your questions, your neurologist's notes if you have them, and whatever your body has been doing lately. However today is going is fine. We'll look together, I'll tell you honestly what I see, and we'll decide side by side what's worth starting with — or whether now is even the right time.

You don't have to arrive with a plan. Come as you are, on whatever kind of day it happens to be.
