When a parent brings a little one in for the first time, I can usually tell from across the room how carefully they're holding them. That carefulness is exactly right. I'd much rather earn it than ask for it — so let me tell you what a first visit actually looks like, in enough detail that it feels familiar before you ever arrive.

Go and press a ripe tomato. Just enough that the skin dimples — and no further. That's the amount. Two more you can try from where you're sitting: press a fingertip until the nail turns pale, then let go; or close one eye and rest a finger on the lid. That's the range we stay within with little ones. It's also why the instrument meets your hand before it meets your child's. A number on a page is easy to read and hard to feel, and I'd like you to feel it.

The gentleness comes from speed. A carpenter swinging a hammer needs a wide arc and a good deal of shoulder; a nail gun does the same work from a trigger held in one hand. What separates them is speed rather than strength. Force is mass times acceleration, so once something moves quickly enough, very little weight is called for — this instrument's impulse travels about a hundred times faster than a hand can move. With children that matters twice over, because little ones are quick. A touch and they're already moving. The tap is finished before the muscles around it have begun to gather.

A newborn and a teenager each get their own setting. The instrument carries a setting ring on its barrel with four positions — Setting 1 for the upper neck, the jaw, small joints of the hand and wrist, and infants, through to Setting 4 for the low back and pelvis. With children, very nearly everything happens at the first two. And the setting is chosen before I pick the instrument up, so it's the same careful amount on a Monday morning as on a Friday afternoon.

It meets your hand first. Before the instrument goes anywhere near your child, I rest it on the back of your hand and let it tap once, so the two of us are starting from the same information. Most parents look a little surprised at how small a thing it turns out to be — and that expression is the one I'm hoping for. It's usually the moment the room relaxes.

We look first, and we look again after. Little ones can't tell us where something feels tight, so we let their body show us instead. Your child lies comfortably face down and we bring the two heels together to compare them. When the pelvis has turned a little to look after a segment that's moving less freely, the leg on that side draws up slightly and reads shorter — the bone is the same length as ever, the muscles on that side are simply holding on. Then a few small movements, each gently loading a different level: a hand behind the low back for L5·L4, shoulders shrugged for T1·C7, one deep breath for T12 and the thoracolumbar junction, chin tucked toward the chest for the occiput. Afterward we compare the heels once more, with you watching. You see how it was, and you see how it is now.

The things parents notice first. Up to the first birthday: the head turning to the same side most of the time, one side at the back of the head flattening a little, settling better upright than lying flat. Once walking begins: one shoe wearing down faster, a walk that leans a little to one side. School age and teens: the head travelling forward over a tablet, shoulder heights looking a little uneven in a fitted shirt, a new stiffness after a growth spurt. These are everyday things, and every one of them is worth a gentle look.
